We’re looking for a compassionate and committed Tailored Care Manager to become part of our supportive, multidisciplinary Tailored Care Management (TCM) team covering Beaufort, Martin, and Washington Counties . This unique opportunity allows you to deliver whole‑person, integrated care—where your work truly matters and makes a lasting impact in the community.

Your Role in Our Mission
As a Tailored Care Manager , you’ll evaluate client needs, develop personalized care plans, and coordinate services that address physical and mental health, social determinants of health, and other life challenges. You will travel throughout Beaufort, Martin, Washington, and surrounding counties , helping close gaps in care and improving lives every day.

Why Join Us?
Earn a competitive salary ranging from $53,000 to $55,000 annually based on experience. This full‑time role offers a Monday–Friday schedule from 8 AM to 5 PM with rotating on‑call responsibilities. We value work‑life balance and provide a supportive environment.

What We’re Looking For
We’re seeking a compassionate, driven professional with a passion for helping others and connecting clients to vital community resources. This role requires flexibility to provide services in various community settings and participate in rotating after‑hours support.

Requirements

Bachelor’s degree in health, psychology, sociology, social work, nursing, or another relevant human services area. Master’s level degree preferred.

Eligible for Qualified Mental Health Professional (QMHP) credentialing.

Minimum 2 years of experience with mental health and/or substance use populations.

Proficiency in Microsoft Office and Electronic Health Records (EHR).

Valid driver’s license, current auto insurance, and a clean driving record.

Strong communication skills and the ability to work independently.
