Role Overview
At Easterseals PORT Health, we’re dedicated to improving the lives, health, and well‑being of individuals and families impacted by substance use and mental health challenges. We’re looking for a compassionate and committed Tailored Care Manager to join our multidisciplinary Tailored Care Management (TCM) team covering Beaufort, Martin, and Washington Counties . This unique opportunity involves delivering whole‑person, integrated care across the region.

Your Role in Our Mission
As a Tailored Care Manager , you’ll evaluate client needs, develop personalized care plans, and coordinate services that address physical, mental, social determinants of health, and other life challenges. You’ll travel throughout the counties and help close gaps in care.

Earn a competitive salary ranging from $53,000 to $55,000 annually. This full‑time role offers a Monday–Friday schedule from 8 AM to 5 PM with rotating on‑call responsibilities. We value work‑life balance and provide a supportive environment.

What We’re Looking For
We’re seeking a compassionate, driven professional with passion for helping others and connecting clients to vital community resources. The role requires flexibility to provide services in various community settings and participate in rotating after‑hours support.

We Also Require

Bachelor’s degree in a field related to health, psychology, sociology, social work, nursing, or another human services area. Master’s level preferred.

Eligible for Qualified Mental Health Professional (QMHP) credentialing

Minimum 2 years of experience with mental health and/or substance use populations

Proficiency in Microsoft Office and Electronic Health Records (EHR)

Valid driver’s license, current auto insurance, and a clean driving record

Strong communication skills and ability to work independently
