Description
We’re looking for a compassionate and committed Tailored Care Manager to join our supportive, multidisciplinary Tailored Care Management (TCM) team covering Beaufort, Martin, and Washington Counties. This unique opportunity allows you to deliver whole-person, integrated care throughout the region.
Your Role
As a Tailored Care Manager, you’ll assess client needs, develop personalized care plans, and coordinate services addressing physical, mental health, social determinants, and other life challenges. This hands‑on role involves traveling throughout Beaufort, Martin, Washington, and surrounding counties.
Why Join Us
You’ll earn a competitive salary of $53,000‑$55,000 annually, based on experience. This full‑time role is Monday‑Friday, 8 AM‑5 PM with rotating on‑call responsibilities.

Qualifications
Bachelor’s degree in health, psychology, sociology, social work, nursing, or related field (Master’s preferred)
Eligible for Qualified Mental Health Professional (QMHP) credentialing
Minimum 2 years of experience with mental health and/or substance use populations
Proficiency in Microsoft Office and EHR
Valid driver’s license, current auto insurance, clean driving record
Strong communication skills and ability to work independently
