Overview POSITION SUMMARY: Provides a variety of support services to homeless or near-homeless Veterans; assists in coordinating services and communication among landlords and CNCAP; provides financial assistance for eligible persons; identifies and accesses resources; seeks to expand program and resources independently and confidently; promotes and maintains public relations; and completes forms accurately and timely. Provide case management, and assist in recruiting and enrolling Veterans into the Supportive Services for Veterans and their Families Program.
Responsibilities Responsible for completing the agency intake form and inputting it into Clarity
Provide ongoing support to veterans and document via home visits, phone calls, and written correspondence with goal attainment of 65% or above
Maintain a non-judgmental attitude toward clients and other CNCAP staff
Ensure clients are assessed through the SSVF process to determine possible assistance through resources
Create family personal plans to develop short and long-term attainable goals with veterans
Ensure files are complete with the necessary paperwork for veteran families participating in the program
Adhere to all CNCAP-related policies and procedures
Promote the program and agency in a positive and professional manner
Qualifications High school diploma or GED required (Bachelor's degree in human services or related field preferred)
Valid driver's license
1-3 years of experience in human services or related field

Extensive knowledge of word, excel, PowerPoint, and databases
Excellent time-management and organizational skills
Demonstrated ability to work with others
Schedule: Monday-Thursday: 7 am - 5 pm or Monday-Friday: 8 am - 4 pm
REPORTS TO: SSVF Case Manager Supervisor
JOB STATUS: Full-Time, 40 hours per week
PAY: $16.50 - $18.25 (depending on qualifications)
