Company Overview And Mission Columbus House opened its doors in 1982 to provide services for men and women at least 18 years of age. Purpose Under the direction of the SSVF Program Manager, the SSVF Case Manager will provide case management services to veteran households who are homeless or at risk of homelessness. Case management services will include housing search, placement, counseling, service coordination, monitoring, and evaluation of program participant progress toward housing goals and linkages to outside referrals.
Duties and Responsibilities Assist and monitor SSVF veteran households progress or housing retention from program entry until successfully discharged.
