About the Job

As part of the SSVF Retention Team, the Case Manager assists clients in breaking the cycle of homelessness using a “whatever it takes” approach. Key responsibilities include the development of Individualized Service Plans (ISP), coordination of housing goals and related supports, and ongoing case management to promote housing stability.

Position Responsibilities include:

Conduct intake and individualized needs assessments for all clients and collaboratively develop ISP addressing housing goals, income goals, and other client-identified needs (medical, mental health, substance use, financial resources, vocational, and social supports).

Provide referrals, linkages, information, and support to resources that help clients achieve ISP goals.

Complete program-specific assessments (e.g., VI-SPDAT).

Evaluate strengths and challenges to address short- and long-term goals; conduct a 90-day review with the client after intake.

Maintain documentation standards as required by the Program and PATH policies, including timely case notes and a complete, accurate, and current client file with all required documents.

Maintain confidentiality of client files per HIPAA and applicable guidelines.

Preferred Qualifications

Two (2) years of experience in healthcare, case management, homeless services, or working with vulnerable populations, or a related field.

Minimum Qualifications

All levels of education and experience are welcome.

Driving is an essential function of this position; candidates must have reliable transportation, a valid driver’s license, and proof of insurance and vehicle ownership for work duties.

Ability to qualify for PATH’s insurance coverage.
