As a Registered Nurse Case Manager, you'll be the bridge between medical care and human comfort guiding patients and families through their journey with empathy, skill, and dignity. You'll collaborate closely with a team that truly values your voice, your expertise, and your heart.

What You'll Bring:
- Graduate of an accredited nursing program (Certificate, Diploma, Associate, or Bachelor's)
- Current RN license in the State of Michigan
- At least 1 year of clinical experience (hospice or home health preferred)
- Compassion, flexibility, and a heart for meaningful work
- Valid driver's license, insurance, and reliable transportation (50% travel required)
- Ability to lift, move, and support up to 50100 lbs. as needed
