Provides linkage and referral to services for tenants and families.
Completes documentation of all tenant related services provided.
Provides weekly/bi-weekly site visits to sites to provide on-going case management services.
Provides on-going monitoring of service delivery which includes routine telephone consultation of tenants.
Participates in case conferences and specific training programs/services with public and private agencies. Obtains at least 12 hours of approved in-service training annually after 36 hours the first year.
Provides information and assistance to tenants.
Develops resident’s assessments and service plan if needed and reviews periodically.
Annual Reporting to HUD.
Maintains an up to date knowledge of senior tenant issues including, Medicaid, Medicare, HUD changes/revisions regarding Case Management and other appropriate community programs.
Maintains confidentiality and process, maintains and utilizes potentially sensitive information in accordance with agency and departmental policies and
Provide representation for benefit eligibility interviews – ie. Medicaid, Food Stamps, Medicare, Social Security etc.
Assist the Site Manager in coordination of presentations on health and educational topics to tenants.
Provide representation to community organizations on behalf of the agency
Provide Information and Referral to community members as needed.
Advocate on behalf of tenant and family.
Independently makes decisions concerning services and supports to seniors considering independent living
Educate tenants, families, and staff on available Community Resources.
Acts as a liaison between community agencies, tenants and families Provides on-going monitoring of service delivery which includes routine telephone consultation of tenants.
Provides crisis management in services as needed.
Compile and maintains Resource Center.
Supervises students, volunteers, aides or other individuals as assigned
Complies with all agency policies and procedures.
Other duties as assigned.
Minimum Qualifications
Bachelor’s degree in social work, gerontology, counseling, psychology or human services, preferred, or without a degree with equivalent years of related work experience.
Two years of social work/services experience or equivalent service coordination and experience with seniors.
Working knowledge of aging process and awareness of community services and programs
Ability to organize, plan and manage caseloads
Must have sound judgment, flexibility, dependability and good writing skills
NYS Driver's License that meets agency policy
Ability to handle multiple tasks simultaneously
Supervisory Responsibilities
Responsible for oversight of students, volunteers, aides or other individuals as assigned
