Care Coordinator Description Screen and intake prospective clients to enter the program with proper forms
Refer new clients for medical services to established pregnancy centers for pregnancy testing, ultrasound, and STD testing
Assist in obtaining government assistance such as WIC, etc.
Assist the Executive Director in providing babies’ essential needs
Coordinate activities supporting clients, including prenatal and parenting education
Determine housing needs, such as rental assistance, and refer to the Executive Director
Provide guidelines, policies, and procedures for clients and obtain their signed agreements
Explain opportunities for continued education or employment and guide clients through the process
Explain Daybreak Ministries’ adoption referral policy to clients
