Job Overview
The Care Manager, or "Designated Care Manager," provides high-quality care and services to residents in assisted living, long-term care, and reminiscence neighborhoods. Responsibilities include delivering hands-on physical and emotional support as outlined in each resident’s Individualized Service Plan (ISP), while maintaining a safe, home-like environment. The Care Manager demonstrates Sunrise’s mission to champion quality of life for all seniors, adhering to all relevant laws, regulations, and company standards.

Essential Duties
Supporting our mission, principles of service, and core values, the Care Manager’s key responsibilities include:
Document resident information accurately in Daily Logs and participate in developing and updating ISPs.
Maintain awareness of residents’ locations and check on them regularly.
Observe, report, and document changes in residents’ conditions.
Participate in daily meetings and notify supervisors of any care needs or condition changes.
Respond promptly to call bells and security alerts.
Greet and communicate professionally with guests, family, residents, and team members.
Assist with activities of daily living, promote independence, dignity, and safety.
Engage residents in life skills, activities, and reminiscence to enrich their daily experience.
Follow safety procedures and maintain sanitation standards.
Additional Responsibilities
Ensure compliance with safety, health, and occupational regulations.
Maintain cleanliness and organization of resident rooms and common areas.
Serve and assist in dining services, following dietary guidelines and ensuring a pleasant dining experience.
Support residents in life skills and social activities, especially in reminiscence neighborhoods.
Participate in team meetings, training, and engagement programs.
Demonstrate core competencies such as calmness under pressure, communication skills, organization, judgment, and problem-solving.
Qualifications
High School diploma or GED; additional certifications as required by law.
CPR and First Aid certifications as mandated.
At least 18 years of age.
Experience working with seniors preferred.
Strong desire to serve and care for seniors.
Ability to make decisions in residents’ best interests.
Valid driver’s license if driving a Sunrise vehicle.
