Job Description: Care Coordinator
Position Overview:
BeeHive Homes is an assisted living facility dedicated to caring for elderly residents. We are seeking a compassionate and organized Care Coordinator to manage the daily operations within one of our homes. Ideal candidates are positive, strong leaders with creative problem-solving skills.
Responsibilities:
Coordinate Care: Manage and coordinate care services by liaising with patients, healthcare providers, and other stakeholders to ensure comprehensive care delivery.
Report Needs to RN: Collaborate with the RN to assess individual needs, goals, and preferences, and develop personalized care plans addressing physical, emotional, and social well-being.
Advocate for Patients: Act as a patient advocate, respecting their rights and ensuring their voices are heard during interactions with healthcare providers.
Service Coordination: Work with RNs and administrators to organize treatments and follow-ups, monitor service quality, and resolve issues.
Documentation and Reporting: Keep accurate records of patient interactions, care plans, assessments, and referrals. Collaborate with RNs to evaluate care effectiveness and identify improvements.
Requirements:
CNA Certification with 2 or more years of experience.
Previous experience in care coordination, case management, or team management preferred.
Strong understanding of healthcare systems, processes, and protocols, or willingness to learn.
Excellent communication and interpersonal skills to build rapport with patients, families, and healthcare professionals.
Ability to handle sensitive and confidential information with discretion.
Exceptional organizational skills with the ability to multitask and prioritize effectively.
