Hours & Schedule
Monday to Friday, 9:00 AM – 5:00 PM with additional on‑call duties on a rota basis.
Key Responsibilities
Manage care workers and perform field‑based duties.
Assess and review individual care plans.
Supervise, instruct, and support care staff in the field.
Maintain accurate records of care delivery.
Coordinate service delivery to meet high‑quality standards.
Qualifications & Experience
Efficient, organized Care Coordinator with a passion for community care.
Experience managing rotas and building strong relationships with care workers.
Previous healthcare experience and solid understanding of CQC, care plans, and rotas preferred.
NVQ Level 3 desirable.
Valid driver’s licence and access to a private vehicle required.
