The Shelter/RRH Stabilization Case Manager will facilitate Scott Place Shelter case management by providing a seamless transition of individuals experiencing homelessness into affordable, quality standard housing. The position will provide a housing-focused plan and implement stabilization services to assure a rapid transition to permanent housing.
Education Associate or bachelor's degree (preferred) from an accredited college or university in Human Services/Social Services related program and/or minimum of 3 years documented case management experience working with homeless individuals, families, and/or homeless service programs is preferred.
Skills While performing the duties of this job the employee occasionally is required to stand, walk, sit, use hands, drive an automobile, talk, and hear. The employee must occasionally lift and/or move 25+ pounds. Must be able to walk up and down at least 35 steps, 4 or more times daily.
Responsibilities Provide own transportation; carry adequate insurance coverage.
Criminal Background Check; Protective Services Record Check; TB Risk Assessment or TB screening by Mantoux method.
Work during the day, evening, and occasional overnights as required by program needs; may include holidays.
Possess excellent computer knowledge and ability to quickly build rapport.
Demonstrate strong written, verbal, and non-verbal communication skills.
Navigate between assertive and supportive communication.
Work as a team member with other staff across the programs.
Maintain a good attitude and behavior with all staff and clients.
Present a professional appearance, be dependable and productive in the work environment.
Maintain confidentiality of client matters and adhere to proper agency, state, and federal privacy guidelines.
Utilize required case management tools and assessments to facilitate self-sufficiency and permanent housing.
Meet with shelter clients no less than one time per week to maintain credibility with the client.
Facilitate appointments that reduce barriers to permanent housing through transportation of clients, appointment scheduling, etc.
Coordinate substance abuse treatment objectives for program participants as needed.
Maintain thorough and organized records of services provided in North Central's Client-Tracking Database (LITT and HMIS) Systems.
Qualifications Valid driver's license. Must obtain CPR/FA cards and any other certifications that may be required for the position/program; to be obtained within the first 3-6 months of accepting position. SOAR certification is required within the 1st year of employment. Some certifications are provided/paid for by the employer. *Social Work License preferred.
