Overview
SASS Therapist/Case Manager (QMHP/LPHA) at Family Service Association of Greater Elgin Area. Reports to the Director of Crisis Services. This is a full-time position (37.5 hours).
Location : Community Based, Kane and DeKalb County
Schedule : Monday–Friday with 2 evenings or Saturday availability
Responsibilities
Clinical: Responsible for completing 15 one-hour appointments per week, including therapy sessions, crisis shifts, and walk-in hours; complete comprehensive assessments, individual treatment plans, and progress notes per agency policy; utilize Greenspace to monitor clinical benchmarks with clients; coordinate services with collaterals, including referral/linkage, discharge planning with inpatient facilities, and advocacy of client rights within education, legal, state, and mental health systems; coordinate services with sub-contracted agencies; respond to emergency/crisis situations with active clients and serve as emergency back-up for SASS Assessments.
Supervision: Discuss cases with supervisor in clinical supervision on a weekly basis; report problem cases and needs for special services and resources to supervisor.
Administrative: Complete all clinical paperwork (IM+CANS, psychiatric referral forms, CIS billing notes, case closing, IM-CAT, SASS transfer forms, and related documentation) and all agency administrative paperwork (time sheets, expense sheets, reports) by deadlines; maintain clinical charts in compliance with applicable rules; attend agency meetings and participate in Utilization Review Meetings; serve on committees as requested.
Community Education/Public Relations: Educate the community about the SASS program and facilitate positive networking relationships with community service providers as assigned.
Training: Maintain clinical charts in compliance with Rule 132 and Rule 140.
Other Duties/Special Assignments/Projects: Develop materials and participate in special community education projects; pursue and maintain licensure/certification as appropriate; perform other duties as assigned; travel as needed; participate in risk management and quality improvement activities; promote collegiality and collaboration in the workplace.
Qualifications
Education: Master’s Degree in Social Work, Counseling, Psychology, or a related field.
Experience: Preferred experience working with youth with mental health issues.
Licensure/Certification: QMHP/LPHA requirements as applicable.
Travel: Must maintain a valid Illinois driver’s license; extensive travel throughout Kane and DeKalb County and occasionally beyond.

Salary and Benefits
Salary range: $47,736 to $67,941.87 per year, depending on experience and qualifications, in accordance with Illinois state law.
