We are seeking a dedicated Nurse Case Manager to join our diverse team in Elgin and be part of our life‑changing mission.
Salary: $72,800 – $78,800 based on qualifications and experience. Hours: Monday – Friday, 8 a.m.–4 p.m.

Responsibilities
Serve as the Administrative and Clinical Coordinator of all nursing activities of two 16‑bed homes.
Administer medications, tube feedings and treatments as prescribed.
Supervise nursing and direct care staff.
Work with staff, parents and regulatory bodies to ensure all appropriate measures are taken in relation to resident care.
Plan, develop and coordinate Quality Improvement Programs to enhance the quality of resident care.
Monitor Infection Control Program, in accordance with policies and procedures, to ensure the highest degree of quality resident care and safety can be maintained at all times.
Qualifications
Valid nursing license in the state of Illinois, current on the Illinois Health Care Worker Registry.
Possess current BLS CPR certification for Healthcare Workers.
Minimum of one year of nursing experience.
Must be able to perform the essential functions of the job and to meet its functional and environmental requirements.
