SASS Therapist/Case Manager (QMHP/LPHA)
Reports to: Director of Crisis Services
Type: Full Time, 37.5 hours
Location: Community Based, Aurora
Schedule: Monday-Friday with 2 evenings or Saturday availability
Summary: Responsible for maintaining an active caseload providing services to clients and their families. SASS follow‑up services will be intensive, short‑term, family and home/community‑based services. SASS therapist/case managers are expected to comply with SASS Standards and Best Practices.
Essential Duties And Responsibilities
Provide 15 one‑hour appointments per week, including therapy sessions, crisis shifts, and walk‑in hours.
Complete agency comprehensive assessments, individual treatment plans, and progress notes per FSA policy.
Utilize Greenspace to monitor clinical benchmarks with clients.
Network with collaterals on behalf of active clients, including referrals, discharge planning with inpatient psychiatric facilities, and advocacy of a client’s rights within education, legal, state, and mental health systems. This includes coordination with sub‑contracted agencies.
Respond to emergency/crisis situations with active clients and provide emergency backup for SASS assessments.
Discuss cases with supervisor in clinical supervision (individual or group) weekly, and report on problem cases, special services, and resource information regularly.
Complete all clinical paperwork, including Integrative Assessments and Treatment Plans (IM+CANS), Psychiatric Referral Forms, CIS Billing Notes, Case Closing, Illinois Medicaid Crisis Assessment Tool (IM‑CAT), SASS transfer forms, and other required documents, ensuring documentation/billing notes are completed within seven days of service.
Meet deadlines for agency administrative paperwork such as CIS Billing Notes, time sheets, expense sheets, and any other requested reports.
Maintain clinical charts in compliance with Rule 132 and Rule 140.
Attend all agency meetings, 9 annual team meetings, clinical supervision, and additional program meetings as required.
Participate in Utilization Review Meetings and serve on agency and/or program committees when requested by supervisor.
Educate the community regarding the SASS program and facilitate positive networking relationships with community service providers and/or networks as assigned by supervisor.
Maintain clinical charts in compliance with Rule 132 and Rule 140 (training responsibility).
Help develop materials and participate in special community education projects/presentations as requested by administrative staff.
Maintain licensure and/or certification appropriate to educational/professional background, and pursue ongoing professional development.
Perform incidental business‑related travel as needed, and contribute to agency Risk Management and Continuous Quality Improvement activities.
Perform job responsibilities with a positive attitude and promote collegiality at the workplace by working collaboratively with others.
Physical and Environmental Considerations
The incumbent will be sitting in a car for travel for several minutes each day. Must be able to lift up to 35 lbs occasionally. The work environment is mostly field‑based and will have varying temperatures and noise levels. The incumbent can expect to work in client homes, hospitals, community agencies, and some office time.
Position Type / Expected Hours of Work
This is a full‑time position, 37.5 hours per week.
Salary Range
Salary range: $47,736 to $67,941.87 per year, depending on experience and qualifications.

Travel
Must maintain a valid Illinois driver’s license. This position requires extensive travel throughout Kane County and occasionally to areas outside of that.
Education And Experience
Must have a Master’s Degree in Social Work, Counseling, Psychology, or a similar field. Preferred experience in working with youth with mental health issues.
