Overview
School-Based Care Coordinators provide care coordination, supportive counseling, and targeted case management services. These services include assessment, service planning, linkage/referral, advocacy, crisis response planning, service plan evaluations, travel and reporting services. They interact with students and provide them with administrative or personal guidance. School-based care coordinators will be located at their assigned school. At school, care coordinators will not only provide services to Westbrook clients and identified at-risk students, but also participate in school activities such as bus duty, lunch duty, and other support throughout the school year.
Responsibilities
Plans and implements programs and activities to reduce school issues.
Maintains high visibility throughout the building and grounds.
Collaborates closely with the school resource officer (SRO).
Coordinates program-related staff development for faculty and staff as indicated.
Monitors student behavior and develops interventions for improvement.
Makes classroom presentations on program-related topics in coordination with instructional staff.
Communicates effectively with students, parents, administration, and staff.
Involves parents and community as “on-site” advocates in the schools.
Serves on the Student Services Team and the School Crisis Team.
Assists the administrative staff with crisis concerns.
Periodically participates in alternative and remediation programs, detention halls, and attendance academies.
Participates in professional growth opportunities.
Completes monthly reports and other accountability reports as assigned.
Makes recommendations to school administration and staff utilizing needs assessment data and observations.
Serves as a team player and role model for other employees; demonstrates a commitment to continuous quality improvement and upholds the organization core values of caring, respect, integrity, responsibility, high expectations, being customer driven, and valuing diversity.
Performs other duties and accepts responsibilities as assigned.
Position requires travel between school and office as needed.

Education and/or Experience
Bachelor's degree in a Human Services discipline (Psychology, Criminal Justice, Regents with health specialization, Recreational therapy, Political Science, Nursing, Sociology, Social Work, Counseling, Teacher education, or Liberal Arts) or a Bachelor’s degree in another discipline with a minimum of 12 credit hours in the human services disciplines listed previously. Prior computer/PC experience using computerized Medical Records and scheduling systems is preferred, but not required. Incumbent must also possess basic math skills. Must pass Criminal Investigation Background and DHHR background check as well as post-offer drug screen.
