Care Coordinator – Westbrook Health Services, Inc.
Position Summary: Responsible for securing and monitoring consumer services as specified by Individual Treatment Plans or emergent situations. Provides assessment, service planning, linkage/referral, monitoring, transportation, and reporting services.
Essential Duties and Responsibilities
Act as Westbrook’s primary contact with consumers.
Coordinate and chair the Interdisciplinary Team to ensure Individual Program Plans are developed and reviewed.
Provide person‑ and family‑centered care to consumers.
Advocate and assist consumers in obtaining needed physical, behavioral, and social services.
Provide supportive counseling services within the scope of training and competencies.
Provide crisis assistance, including developing a personal crisis safety plan.
Ensure transportation needs are met, including transport to appointments.
Provide care coordination across settings (office, home, community).
Record documentation of services in electronic health record.
Assist consumers in maintaining eligibility for applicable programs, monitor financial status, and conduct required evaluations.
Assist with Continuous Quality Assurance and participate in Utilization Management.
Comply with federal, state, and company policies, procedures, and regulations.
Maintain confidentiality and comply with HIPAA regulations.
Demonstrate knowledge of and support Westbrook Health Services mission, vision, values, policies, procedures, confidentiality standards, and code of ethical behavior.

Education and Experience
Bachelor’s degree in a Human Services discipline (Psychology, Criminal Justice, Health Specialization, Recreational Therapy, Political Science, Nursing, Sociology, Social Work, Counseling, Teacher Education, or Liberal Arts) or a Bachelor’s degree in another discipline with a minimum of 12 credit hours in the human services disciplines listed above. Experience with computerized medical records and scheduling systems preferred. Basic math skills required. Must pass a Criminal Investigation Background check, DHHR background check, and a post‑offer drug screen.
Physical Requirements
Requires ability to talk, hear, sit, use hands and fingers, stand, walk, reach, climb or balance, stoop, kneel, or crouch. Vision required: close vision. Must be able to travel within the catchment area. Work is primarily in an office environment.
