RN - Nurse Care Coordinator

Pay range: $62,000 - $78,000 per year (based on skills and experience).

Position Type: Full Time (40 Hours)
Responsibilities
Perform required nursing assessments to identify individuals with identifiable risk factors.
Act as liaison between public and private healthcare facilities, and service agencies to carry out referral instructions.
Participate in interdisciplinary treatment team meetings, cooperate with and utilize resources of other community agencies to achieve treatment goals.
Incorporate measurable, disease‑specific goals into treatment plans; evaluate progress and communicate to the treatment team.
Make home visits to administer or assess clients' condition or provide services in their environment as appropriate.
Provide crisis intervention, supportive interaction, and education to foster self‑management and accountability.
Accompany clients to medically complex appointments.
Receive clinical supervision from the Nursing Supervisor and participate in periodic review.
Assist the Nursing Supervisor in maintaining clinical staffing within the medication clinic.
Work full‑time (40 hours per week) during normal business hours.
Qualifications
Graduate of an accredited nursing program and valid RN license in Ohio.
Experience providing nursing services to persons with severe and persistent mental illness in community and clinic settings.
Valid Ohio driver’s license and vehicle; insurability through agency insurance carrier.
Knowledge of nursing rules and regulations in Ohio.
Excellent organizational, attention to detail, and communication skills.
Ability to work in a team environment.
Commitment to confidentiality and HIPAA compliance.
Proficiency in Microsoft Office.
Skill in managing time and meeting deadlines.
Experience with crisis stabilization techniques.
Ability to work non‑judgmentally with diverse individuals.
