This is a Full-Time position that will help the growth of our new hospice division.

What You Will Do as a Hospice RN Field Nurse

As a Hospice RN Field Nurse , you will provide support to patients and families to ensure a safe and comfortable environment.

Primary Responsibilities

Completing an initial assessment of the patient and family to determine hospice care needs; providing a complete physical assessment and history of current and previous illness.

Developing a care plan, establishing goals based on nursing diagnosis and incorporating therapeutic, preventive, and rehabilitative actions.

Re-evaluating and revising patient care needs as needed.

Administering medications and treatment safely and as prescribed by the physician.

Counseling and educating patients and families on meeting nursing and related needs.

Providing health care instructions to the patient as appropriate per assessment and plan of care.

Participating in on‑calls.

This position is open to both PRN and FT candidates.

Education, Licensure & Certification Requirements

Graduate of an accredited school of nursing

Active RN license

One (1) to two (2) years of recent acute care experience

Bachelor’s degree with one (1) year of hospice/home health experience preferred
