Job Summary
The RN Case Manager is a member of the interdisciplinary team who works to meet the physical, medical, psychosocial, emotional, and spiritual needs of hospice patients and families facing terminal illness and bereavement. The RN Case Manager provides coordination of care, ensures continuous assessment of each patient's and family's needs, and implements the interdisciplinary plan of care. This position is hands-on and requires supporting the Connecticut Hospice's vision, mission, and operating principles.

Essential Duties and Responsibilities
Work as part of the interdisciplinary team to meet the holistic needs of hospice patients and families.
Coordinate and manage the hospice plan of care.
Assess, plan, implement, and evaluate ongoing care needs, collaborating with other disciplines.
Supervise care provided by licensed practical nurses, home health aides, and volunteers.
Coordinate services with all disciplines and community resources.
Provide education and counseling to patients and families.
Participate in on-call rotations, bereavement support, and quality improvement activities.
Maintain proper documentation and communication with facilities and team members.
Health & Safety
Report and manage injuries, incidents, and emergencies following policies.
Ensure safety and compliance with all safety policies and procedures.
Qualifications
Current CT RN License in good standing.
Minimum of one year experience in hospice, home care, or long-term care.
Valid CT driver’s license and auto insurance.
Excellent communication skills and attention to detail.
Computer Skills
Proficiency in Microsoft Office and ability to operate media and office equipment.
Physical Requirements
Ability to perform physical tasks such as lifting, bending, and walking.
Good vision, hearing, and endurance to perform duties effectively.
