Position Overview The RN Case Manager is a direct link between medical services and the ICF/IRA residents and staff, as well as with clinical professionals outside the program. The RN Case Manager is responsible for the complete nursing care and performance of all routine medical procedures within the scope of the nursing practice. The RN Case Manager supervises LPNs, AMAPs, and Direct Support Professionals in nursing tasks and activities. The RN must possess working knowledge of OPWDD rules and regulations and JBFCS policies and procedures. The RN must be capable of supervising, understanding, and working effectively with diverse populations.
Key Essential Functions Provide direct nursing care as prescribed by the physician/Nurse Practitioner, in compliance with recognized nursing standards and State and Federal regulations.
Use nursing process to assess the total health needs of individuals with developmental disabilities.
Maintain direct interaction with consumers regularly (at least weekly).
Perform nursing assessments, including physical examinations for each consumer upon entry into the program and quarterly thereafter.
Develop and implement an individualized plan for nursing service; update the plan at least annually or with significant changes in condition.
Coordinate all acute and ongoing health services as needed and mandated by regulation.
Monitor health and behavioral status, labs, and efficacy of treatments; communicate findings to appropriate clinical personnel.
Ensure accurate, timely, outcome-based medical record documentation in nursing notes.
Provide annual training to staff in nursing tasks, protective and preventive health measures (e.g., health hygiene, infection control, illness signs, first aid, seizure management, client-specific medical issues).
Ensure training is provided to all new hires and per diems before client-specific assignments.
Provide training and ongoing support to direct care staff regarding medication administration, tube feeding, and diabetic care using the OPWDD-approved curriculum.
Conduct annual clinical performance evaluations for unlicensed direct support staff for procedures including medication administration, tube feeding, and insulin administration; include evaluations in the employee’s annual performance review.
Maintain documentation of all training curricula, attendance, and post-training testing.
Experience and ability in working with electronic health records, electronic care coordination, and EHR interface with external providers.
Active participant in the Interdisciplinary Team (IDT).
All other duties as assigned.

Educational/Training Required Active, unrestricted New York State license as a Registered Professional Nurse.
Bachelor’s degree and a minimum of 3 years clinical experience in a hospital or primary care setting preferred.
Experience/Language Preference Experience with the intellectually disabled population preferred.
Managerial experience preferred.
Excellent verbal and written communication skills; ability to work in a team and independently with sound judgment.
Computer Skills Proficiency in Microsoft Office Suite applications required.
Work Environment and Physical Requirements The work environment involves no physical risk or hazardous conditions.
Typical physical requirements: sit approximately 50% of the time and stand approximately 50% of the time.
