Program Description:
The Supportive Housing (Scattered Sites) is a 128-bed unit designed as permanent housing for individuals, some with families, who have a mental health diagnosis and can benefit from supportive services. Support services are provided either in the individual’s home or at the Supportive Housing Office. Residents have access to the program’s 24-hour on-call system. The services include an assigned case manager in a collaborative, goal-oriented manner allowing for client choice and respecting individuality and independence.
Essential Responsibilities and Role of a Case Manager:
The Case Manager is responsible for the case management of the tenants and clients in their caseload. This includes ensuring that case records are timely and complete, and that all interagency and collateral contacts are indicated in the charts. Progress notes should reflect work toward goals, having both theme and continuity.
Activities of Case Managers are aimed at linking the tenants in Supported Housing to the service system, community resources, and support systems, and at coordinating various services for the tenant to achieve successful independent living. The objective of case management is the continuity of care and service to each tenant in the Supported Housing Program, while meeting the standards, philosophy, and policies of Geel Community Services in accordance with NYS Office of Mental Health (OMH) and NYC Department of Health and Mental Hygiene (DOHMH) guidelines.
Provide case management services for 18 to 20 tenants in a supportive housing program.
Available to tenants in caseload based on need for regular contact and/or upon request of the tenant when appropriate. Contacts with tenants pertain to face-to-face meetings at the office, apartment visits, outreach telephone calls, and letters, and outside appointments.
Accessible to be on call for 24-hour crisis intervention.
Qualifications and Requirements:
Education: Preference for a BA or higher in Social Work or a related human services field, but will consider HS Diploma or Equivalency.
Experience: Preference for at least two years’ experience working with this population or related human services field with a BA, or three years’ experience working with this population with HS diploma or GED.
Physical Requirements:
Must be able to access apartments above the first floor with or without elevator access.
Must be able to travel to different locations using public transportation.
