Position: Residential Case Manager (RCM) –RISE UP
Under the supervision of the Executive Director, the RCM is responsible for home management, case management, staff supervision and mentoring, training staff, advocacy, health coordination, and promoting consumers’ independence across all aspects of aging. Responsibilities include helping consumers create and maintain a home, coordinating plans for home life, relationships, work, and spiritual life; ensuring health and safety; and maintaining compliance with policies, procedures, and funding requirements. The role may include direct consumer support, scheduling, after-hours on-call rotation, and other duties as assigned.

Responsibilities

50% A. Supervision of staff, volunteers and students

Schedule staff to carry out daily operations and oversee team activities related to participant status changes.

Communicate program goals, policies, and procedures to staff; keep teams informed of agency issues.

Develop policies and position descriptions; participate in recruitment and onboarding; mentor staff.

Conduct performance reviews, coaching, and disciplinary processes in coordination with leadership.

Ensure staff are educated on Abuse/Neglect Policy, Confidentiality, Emergency Planning, and Safety.

Maintain details of each consumer’s home life (shopping, banking, meal planning, chores, medication coordination, etc.) and health/safety standards.

Organize quarterly team meetings and ensure consumer-centered approaches are followed.

Maintain regular communication with the Executive Director and Office Manager.

50% B. Case Management and Advocacy

Coordinate with families, guardians, healthcare providers to support medical, social, and vocational development.

Complete residential support plans, life skills assessments, risk and safety plans, and documentation for Rise Up, Dane County, or funding sources.

Facilitate meetings around Behavioral Plans and consumer-focused home meetings.

Accompany consumers to routine and acute medical appointments and coordinate health monitoring.

Advocate on consumers’ behalf and support home life and community integration.

Lead home support teams to ensure satisfaction and meaningful connections.

50% C. Consumer Management

Manage personal funds for each consumer, maintain records, and file required tax forms.

Oversee medications, coordinate with healthcare providers, guardians, and staff, and communicate medical updates promptly.

Maintain eligibility for Medical Assistance, Food Share, or equivalents.

Maintain consumer files and confidentiality at all times.

50% D. Direct Consumer Support in General and in Emergency Situations

Provide direct community-based support, assist with home upkeep, shopping, meals, and transportation as needed.

Serve as on-call scheduler/Administrator on call; respond to after-hours health and safety needs.

Gather information as a Fact Finder and support relationship-building for consumers.

Assist consumers in developing friendships and family connections; support hospitality in homes and neighborhoods.

50% E. Other

Attend required meetings, trainings, and complete health screenings; maintain documentation and logs.

Participate in retirement and end-of-life planning discussions when applicable.

Support mission, engage in professional development, and empower consumers to direct their own lives.

Build cooperative relationships with consumers, families, guardians, medical professionals, and staff; perform other duties as assigned.

Position Qualifications
Requirements include a combination of knowledge, skills, and abilities, including but not limited to:

Bachelor’s degree in health or human services or an equivalent combination of training/education/experience (four years total) with at least one year in working with people with disabilities; case management experience preferred.

Experience with health needs changes and staff leadership; supervision experience preferred.

Proficiency with computer applications (Word, Excel, MS Office suite); ability to type and manage documentation.

Effective conflict management, customer service, and communication skills.

Ability to work independently and as part of a team; manage multiple priorities in a fast-paced environment.

Ability to coordinate procurement of durable medical equipment and to manage financial, medical, and household tasks.

Valid State of Wisconsin driver’s license, reliable transportation, and insurance.

Salary: $55,000 per year
