We're hiring! Transitions At Home , a non‑profit home health agency serving Wisconsin, is currently seeking a full‑time RN Case Manager in the Madison area.

In this role, you’ll coordinate and oversee patient care in the home setting, ensuring personalized, high‑quality outcomes for every client.

What Does a Transitions at Home RN Case Manager Do?

Provide skilled nursing care, including assessments, medications, treatments, and coordination of services in the client’s residence.

Advise and counsel patients and their families or significant others according to the patient’s physician orders and plan of care.

Work under the direction and supervision of the RN Clinical Manager and Administrator.

Hours

40 hours per week, Monday through Friday, 8:00 am - 4:30 pm.

No night shifts.

Limited on‑call rotation.

Qualifications / Job Requirements

Active Wisconsin Registered Nurse license.

Two (2) years of nursing experience, including at least one year in home health (preferred).

CPR certification.

Valid Wisconsin driver’s license and car insurance.
