Position Summary
We have an exciting opportunity to join our team as a Registered Nurse - Wound Ostomy Care Coordinator - per diem. Coordinates the clinical care for patients at risk for or have actual impaired skin integrity, wounds, pressure injuries & ostomies. Responsible for the assessment, planning, evaluation, and execution of wound & ostomy care in the hospital. Is an advocate for prevention of pressure injuries and implements plans to safeguard patients at risk. Guides the nursing staff on the appropriate products and treatments for patients with mild to complex wounds.

Job Responsibilities

Consults and evaluates skin/wounds for inpatients.

Assists staff in providing care to patients with or at risk of developing wounds.

Suggests proper support and positioning surface products.

Monitors use of rental support surfaces.

Advises staff on available and appropriate wound care products.

Evaluates patient’s progress documenting in chart and communicating to nursing staff.

Provides information to staff and patients regarding management of ostomy or continent diversions.

Assists patient and/or family in ostomy or continent care plan.

Provides support and aid to patient and staff in plan to stabilize tube, protect skin at site and provide advice for suitable containment of drainage.

Helps staff manage patients with urinary and fecal incontinence.

Assesses patient for causative factors; suggests referrals if indicated and aids in developing plan of care.

Participates in PI not limited to Nursing Department and may include interdisciplinary projects.

Maintains records and statistics and submits monthly.

Participates in patient billing, coding and documentation as needed.

Analyzes stock and non-stock items and recommends appropriate additions, deletions to assure cost effectiveness and quality of products used.

Builds and maintains a positive working relationship with community referral sources.

Participates in hospital committee meetings.

Provides relevant feedback to managers regarding their staff performance and patient care issues.

Provides orientation to new staff.

Collaborates with the Director of Professional to establish ongoing education and annual competencies.

Serves as clinical preceptor for nursing students and new registered nurses.

Minimum Qualifications
To qualify you must have a Graduate of an accredited School of Nursing.

NYS RN license/current registration.

BSN required with current Wound Ostomy and Continence Nursing (WOCN) Certification OR MSN with Wound Care experience.

Current Basic Life Support (BLS) Certification from the American Heart Association (AHA).

Minimum of five years of nursing experience.

2 years leadership and education experience (can be combined).

Required Licenses
Registered Nurse License - NYS.

Preferred Qualifications
Masters Degree preferred.

NYU Langone Hospital—Suffolk provides a salary range to comply with the New York state Law on Salary Transparency in Job Advertisements. The salary range for the role is $40.67 - $61.54 Hourly. Actual salaries depend on a variety of factors, including experience, specialty, education, and hospital need. The salary range or contractual rate listed does not include bonuses/incentive, differential pay or other forms of compensation or benefits.
