Western Tidewater CSB is seeking qualified Case Managers for various departments throughout our organization. ID/DD experience is preferred for this position. Responsibilities and Duties for ID/DD Case Manager: Responsible for consumer assessment, service linkage, coordination, development, and implementation of a case management ISP
Assist and provide follow up instruction, education, and counseling to consumers/families with ID/DD
Monitor service delivery through contacts with consumers receiving services, family members, service providers, and periodic site/home visits
Manage consumer records according to all Licensure and Medicaid standards.
Qualifications and Skills for ID/DD Case Manager: BA/BS from an accredited college or university with major course experience in Psychology, Social Work, or related field
One year of clinical experience in a Human Services field
QMHP or QIDP preferred
KSA’s (Required)
Valid Driver’s License with an acceptable DMV record at the time of employment and continuously thereafter
