Overview Community South Hospital has a PRN RN Case Manager position available. The Registered Nurse Case Manager is responsible for the assessment, planning, implementation, coordination, monitoring and evaluation of services across the continuum of care to ensure quality patient outcomes and appropriate utilization of health care services. Responsible for supporting the healthcare team towards a smooth transition from one level of care to another in support of the patient/family. To provide ongoing support and expertise through comprehensive assessment, planning, implementation, and overall evaluation of individual patient needs. The overall goal of the position is to enhance the quality of patient management and satisfaction, to promote continuity of care and cost effectiveness through the integrating and functions of case management, utilization review and management and discharge planning. Has accountability for the care, coordination, and discharge planning of inpatient and observation services.
Responsibilities Provide ongoing assessment, planning, implementation, and evaluation of individual patient needs across the continuum of care.
Support the healthcare team to ensure smooth transitions between levels of care for patients and families.
Coordinate discharge planning and ensure continuity of care and appropriate utilization of services.
Engage in case management activities including utilization review and discharge planning for inpatient and observation services.
Qualifications Graduate of a nursing program accredited by the National League for Nursing, Commission on Accreditation of Education in Nursing (ACEN), or Commission on Collegiate Nursing Education (CCNE).
Licensed as a Registered Nurse by the Indiana Professional Licensing Agency (required).
Two years of recent clinical experience or three years of recent hospital case management experience (required).
Certification in Case Management (CCM or ACMA) (preferred).
Strong teamwork, organizational, and problem-solving skills; ability to work independently and within a network team.
Flexibility in work schedules and coverage at any site; ability to establish rapport with a wide variety of people.
Strong computer skills.
