Location: New York City (Brooklyn, Bronx, Manhattan, Queens, and Staten Island)

Job Type: Full-Time | Hybrid Field & Office Role

Salary: $90,000 per year + Full Benefits

As a Registered Nurse (RN), your clinical expertise can go beyond bedside care. At Doral Health & Wellness, we invite you to join our mission-driven care management team supporting New Yorkers living with serious mental illness, chronic conditions, and complex social needs.

This hybrid field-based role allows you to work in the community and help clients thrive by connecting them with the right services at the right time.

What You'll Do As An RN Care Manager

Coordinate medical, behavioral, and social care for clients across NYC boroughs
Conduct in-person assessments and health screenings
Develop and monitor individualized care plans
Refer clients to housing, behavioral health, substance use, and other support services
Advocate for clients across health systems and community programs

Who We’re Looking For

Current NYS RN license
Bachelor’s Degree in Nursing or equivalent experience
1–2 years of clinical experience, preferably in behavioral health or community care
Strong assessment, documentation, and communication skills
Comfortable working in the field across NYC boroughs
Spanish or other second language proficiency is a plus

Available RN Roles

Health Home Plus (HH+) – for adults with SMI or substance use disorders
Assisted Outpatient Treatment (AOT) – court-mandated mental health treatment
HCBS/HARP Assessor – eligibility evaluations using UAS-NY
HIV/AIDS Program – care management for people living with HIV
