Job Overview Join Hillsdale Hospital as a Registered Nurse or LPN Case Manager in Home Care. This is a part-time role, 24 hours per week, including weekend and call rotation.

Qualifications For The Home Care Nurse
High school diploma or equivalent
LPN diploma / certificate, or associates degree or BSN for an RN
Valid Michigan RN or LPN license
Minimum of one year of current, continuous experience in an acute care setting is preferred
Must have a dependable vehicle with current driver's license and auto insurance coverage
BLS certification (available through Hillsdale Hospital)
Proficient with computer software used in healthcare
Responsibilities Of The Home Care Nurse
Calls patients to schedule visits
Evaluates patients' and residents' health and implements plan of care in collaboration with the patient, family, and physician
Administers treatment, medications and IV solutions according to physician’s orders
Observes and evaluates symptoms and reactions to treatment; initiates corrective action if needed, updating plan of care and advising physician of changes in patient's condition
Documents and maintains records and reports regarding nursing care provided
Completes OASIS information and performs the comprehensive assessment as SOC, resumption, transfer, death in the home and at discharge
Coordinates services to facilitate holistic patient care
Maintains daily visit productivity of 5 to 7 visits adjusted for travel and other assignments
