Overview Job Description Summary – The Registered Nurse Case Manager plans, organizes and directs care and is experienced in nursing, with emphasis on community health education/experience. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individuals and families within their homes and communities.
Responsibilities Completes an initial, comprehensive assessment of patient and family to determine hospice needs, including a complete physical assessment and history of current and previous illness(es).
Provides professional nursing care by utilizing all elements of the nursing process as defined in the state Nurse Practice Act.
Assesses and evaluates patient status by writing and initiating the plan of care; regularly re-evaluates patient and family/caregiver needs and revises the plan as necessary.
Initiates the plan of care and makes necessary revisions as patient status and needs change.
Uses health assessment data to determine nursing diagnosis; develops a care plan with goals based on nursing diagnosis and palliative nursing actions, including patient and family in the planning process.
Initiates appropriate preventive and rehabilitative nursing procedures; administers medications and treatments as prescribed by the physician in the plan of care.
Counsels the patient and family in meeting nursing and related needs and provides health care instructions as appropriate per assessment and plan.
Assists the patient with activities of daily living and facilitates the patient’s progress toward self-sufficiency and comfort care.
Maintains current documentation on the EMR system according to organization policies/procedures and applicable laws/regulations.
Coordinate patient care for an assigned caseload; may act as Admission Nurse or Case Manager as assigned by Clinical Manager/DPCS, coordinating with intake, program staff and management for initial admission/assessment and plan of care.
Communication Completes and submits accurate clinical notes regarding patient’s condition and care; records pain/symptom management changes/outcomes as appropriate.
Communicates with the physician regarding patient needs and reports changes in condition; obtains/receives physician orders as required.
Coordinates with community health-related persons to align the care plan; teaches patient and family self-care techniques and provides medication, diet and other instructions as ordered by the physician.
Works with the interdisciplinary group to provide and maintain a safe environment and continuity of care; attends interdisciplinary group meetings.
Additional Duties Participates in on-call duties as defined by policy.
Ensures arrangements for equipment and other necessary items and services are available.
Supervises ancillary personnel and delegates responsibilities when required.
Maintains professional growth and development through continuing education and in-service training.
Fulfills assigned case responsibilities and participates in quality assessment/performance improvement activities.
Adheres to attendance and reliability guidelines as essential for quality and consistent patient care.

Job Qualifications Graduate of a National League for Nursing accredited nursing school; current licensure in state of practice and CPR certification.
Must possess a valid California Driver’s License with proof of auto insurance.
May have an RN license and prior LVN experience in public health, home care, Hospice, Hospital or Skilled Nursing Facility (California only).
All States: RN with at least 1 year of professional nursing experience within the last 3 years or a baccalaureate degree in nursing from an NLN-accredited program; preferred in public health, home care, or hospice nursing; valid RN license; driver with insured vehicle.
Self-directed, able to work with minimal supervision; excellent communication and problem-solving skills; able to lift and transfer patients and perform required nursing skills.
Physical Requirements Ability to sit, stand, drive, walk, bend, kneel, crouch and crawl as needed; able to lift up to 50 pounds frequently and greater than 50 pounds occasionally; push and pull up to 50 pounds frequently.
