Cheer Home Care is hiring a Care Case Manager in San Diego. The Care Case Manager creates personalized care plans, manages care, and provides feedback to clients, their representatives, and caregivers. They work closely with our office team and should have excellent communication and problem-solving skills.
Requirements
Upbeat Personality
Master's degree in Social Work, Nursing, or relevant field preferred
Industry experience preferred
Strong computer skills
Strong organizational and communication skills
Pass Live Scan Fingerprint Clearance (LIC 9163)
Valid driver's license and proof of insurance
Cell phone with internet access
Proof of negative TB results
Willing to work one weekend a month as backup on-call
