Job summary and qualifications: This is a full-time Day Shift position (typically 8:00 am – 4:30 pm, Monday – Friday with some weekends).

Responsibilities

Promote patient-centered care by coordinating the plan of care for the patient stay, managing length of stay, ensuring appropriate resource management, and developing safe discharge plans in collaboration with the multidisciplinary team.

Facilitate progression and transition of care using established criteria in conjunction with the multidisciplinary team.

Coordinate activities that promote quality outcomes and patient throughput while balancing optimal care with appropriate resource utilization.

Perform a comprehensive assessment of psychosocial, medical, and discharge needs of patients/families and identify available resources.

Reassess clinical condition as indicated; develop strategies to mitigate readmission risk, including education on healthcare resources and community-based resources.

Coordinate the plan of care and discharge plan with the healthcare team and the patient’s physician to facilitate a successful care transition.

In partnership with Social Services, ensure post-acute medical needs and level of care are appropriate; refer to Social Services when psychosocial risk factors are identified.

Engage patient and family in identifying needs and realistic goals; communicate with physicians, hospital staff, and outside agencies.

Evaluate progression of care using InterQual criteria; escalate issues through the chain of command as needed.

Make referrals to third-party payers, disease and case management programs for recurring or chronic disease patients.

Support patient throughput and safe discharge/transition through effective care coordination.

Document professional recommendations, discharge plan, and case management activities for the health care team.

Align patient needs with available resources to ensure safe discharge/transition.

Advocate for patient rights, values, and culturally competent care; respect patients’ ethnic, cultural, or religious values.

Qualifications

Registered Nurse with current TX state license required.

Associate Degree in Nursing or Nursing Diploma required; Bachelor’s Degree in Nursing preferred.

3+ years of clinical hospital nursing experience required or 2+ years of case management experience required.

Certification in case management preferred; InterQual experience preferred.
