Regional Wound Care Coordinator - LTC
Located throughout New York City & Long Island, NY
Salary: $120K-$160K range based on experience

Qualifications:
Must have New York RN license
Proficiency in managing wounds in a nursing home setting
Prior nurse management/supervisory experience in long term care
Familiarity with discharge planning processes to facilitate smooth transitions for patients.
Understanding of family planning considerations as they relate to patient education.
Strong organizational skills for maintaining detailed medical records.
Expertise in critical care nursing principles applicable to wound management.
Competence in pain management strategies to address patient discomfort during treatment.
Must be willing to travel
Responsibilities:
Develop individualized care plans based on assessment findings and evidence-based practices.
Perform and oversee wound care treatments, including dressing changes, debridement, and application of topical agents.
Collaborate with physicians and other healthcare professionals to coordinate comprehensive patient care.
Educate patients and families on wound care techniques, infection prevention, and self-management strategies.
Maintain accurate medical records documenting assessments, interventions, and patient progress.
Participate in discharge planning to ensure continuity of care post-hospitalization.
Stay updated on the latest advancements in wound care management and participate in ongoing professional development.
