Position Summary

We are seeking a dynamic and forward-thinking Pharmacy Manager, Clinical Integration to spearhead the development, implementation, and cross-organizational integration of innovative clinical pharmacy services. This role will focus on expanding services such as medication adherence, transitions of care, and ambulatory care pharmacy in alignment with organizational goals and population health initiatives.

The ideal candidate will be a strategic thinker with strong project management skills, clinical expertise, and the ability to foster collaboration across departments, care settings, and partner organizations.

Key Responsibilities

Clinical Program Development & Expansion

  • Design, implement, and optimize comprehensive clinical pharmacy services, including but not limited to:

    • Medication Adherence Programs

    • Transitions of Care Services

    • Ambulatory Care Services

  • Conduct needs assessments, identify gaps in care, and develop tailored clinical pharmacy initiatives.

  • Utilize evidence-based practices to support clinical service models that improve patient outcomes and reduce hospital admissions.

Cross-Organizational Leadership & Collaboration

  • Collaborate with interdisciplinary teams to ensure successful integration of clinical pharmacy services across Oak Street Health.

  • Serve as a liaison between Oak Street Health pharmacy, Oak Street Health care teams, and other cross-organizational partners.

  • Align clinical pharmacy services with organizational quality improvement, population health, and value-based care initiatives.

Implementation & Project Management

  • Lead the end-to-end project lifecycle for new clinical programs, including planning, stakeholder engagement, resource allocation, execution, monitoring, and evaluation.

  • Develop and maintain standard operating procedures (SOPs), workflows, and documentation templates.

  • Monitor key performance indicators (KPIs) and clinical outcomes; generate and present performance reports to leadership.

Qualifications

Education and Licensure

  • Doctor of Pharmacy (PharmD)

  • PGY1 residency preferred

Experience

  • Minimum 3–5 years of clinical pharmacy experience, including implementation of new services.

  • Demonstrated success in cross-functional leadership and program development.

  • Experience working in a value-based or population health-driven care model preferred.

Skills and Competencies

  • Strong clinical knowledge in ambulatory care, chronic disease management, and care transitions.

  • Proven project management and organizational skills.

  • Excellent interpersonal and communication abilities.

  • Proficiency in data analysis, clinical documentation, and reporting tools (e.g., Epic, Excel).

  • Ability to navigate and lead change in complex healthcare systems.

Pay Range

The typical pay range for this role is:

$141,625.00 - $239,250.00


This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above. This position also includes an award target in the company’s equity award program.
