We are hiring a Patient Care Manager RN with Home Health experience.
The Home Health Patient Care Manager and RN Hybrid is responsible for supervising and coordinating clinical services, providing and directing nursing care to patients in their homes as prescribed by physicians and in compliance with applicable laws, regulations, and agency policies. The role involves coordinating and supervising an interdisciplinary team to ensure high-quality care for home health patients in accordance with physician plans of care and all relevant laws and regulations.

Responsibilities
Provide clinical services within your scope of practice, as defined by state laws, in accordance with the plan of care and in collaboration with the healthcare team
Receive referrals, assign clinicians appropriately, evaluate patient orders, and coordinate start of care visits
Determine patient home health benefits, medical necessity, and secure ongoing insurance approvals
Continuously assess patient needs, ensure individualized and appropriate care that meets eligibility criteria and physician orders
Review assessments and care plans daily, consult with clinicians as needed
Education and Experience
Current RN licensure in the state of practice
Current CPR certification
Valid driver's license, vehicle insurance, and access to reliable transportation
