Overview
We are looking for a high‑energy, compassionate, detail‑oriented Registered Nurse (RN) to serve as a Hospice Case Manager for our patients. Our Hospice RN Case Managers plan, organize, and direct hospice care utilizing the nursing process of assessment, planning, interventions, implementation, and evaluation. They effectively interact with patients, caregivers, families, and interdisciplinary team members while maintaining professional nursing and clinical competency standards.

Responsibilities
Respond to clinical referral information with care and expertise, addressing inquiries and requests with compassion.
Collaborate closely with the Clinical Director to assess client eligibility and suitability for specialized hospice services.
Act as a source of comfort and support for patients, ensuring their comfort and quality of life remain paramount.
Apply nursing skills by delivering top‑notch care rooted in proven principles and techniques to ease pain and bring solace to patients.
Empower patients and families through education, encouraging active participation in personalized care plans aligning with their goals.
Share responsibility for call duties, ensuring presence when it matters most.
Qualifications
Current, unencumbered RN license.
Minimum one (1) year experience as an RN in a medical‑surgical/acute care setting; hospice experience is a plus.
Computer proficiency, including typing and EMR background.
Current CPR certification if required by state.
Compensation
$89,000 - $96,000 per year (Average Pay Range). The pay range listed is a general guideline and not a fixed offer. Final compensation will be determined based on the candidate’s experience, qualifications, and the specific responsibilities of the position, and will be confirmed at the conclusion of the interview process.
