Position Summary:
The Part-time Registered Nurse Case Manager will report directly to the Director of Nursing.They will help ensure that our clients receive safe, professional, and loving care in their homes. The Part-time Registered Nurse Case Manager will complete in-home clinical assessments, provide supervision of aides and provide guidance and training to caregivers to ensure the highest quality of care for our patients.
Key Responsibilities:
· Conduct In Person patient interviews and comprehensive physical assessments.
· Oversee the implementation and ongoing assessment of the patient’s plan of care through the management of home health aides, LPNs, RNs, and other caregivers.
· Communicate patient conditions and collaborate with appropriate providers to deliver care when patient needs evolve.
· Provide education to patients and families on proper home health care procedures. Ie. Wound care, IV administration, medication management.
· Work to decrease readmissions by promoting preventive care and ensuring continuity of care.
· Document patient information accurately and maintain medical records.
· See patients and clients, as needed.
· Auditing charts weekly.
· This position requires frequent standing, walking, bending, and lifting up to 50 lbs, with occasional assistance in patient transfers.
· Perform other duties as assigned.

Education:
· Associate or Bachelor's degree in a Nursing (Required).
Qualifications:
· Unrestricted Licensed IN Registered Nurse (RN) is required.
· One (1) year of professional experience practicing as a Registered Nurse (RN) in home health; previous case management/utilization review experience preferred.
· Must be able to drive regularly, climb stairs, and tolerate varied home environments (e.g., temperature, allergens)
· Must have an active driver license and reliable transportation to drive throughout the day / territory.
· Fine motor skills, visual acuity, and auditory perception are required for patient assessments and documentation.
· Extensive OASIS experience and knowledge with a demonstrated ability to oversee and ensure compliance with OASIS guidelines.
· Ability to effectively communicate with patients, families, and healthcare professionals.
· Proficient in conducting physical examinations and assessing patient conditions.
· Strong knowledge of medical terminology and pharmacology.
· Ability to interpret medical test results and develop appropriate care plans.
· Exceptional critical thinking and problem-solving abilities.
· Medicare, ACHC and/ or CHAP experience preferred.
· US work authorization (Required)
· Must have your own computer and high-speed internet. We use MS Teams for regular video calls.
