Responsibilities

Assess and monitor the physical, emotional, and psychological needs of patients

Create hospice care plans that align with the patient’s wishes and goals

Direct nursing care: administering medications, treatments, and interventions

Provide pain and symptom management

Educate and support the patient’s family and caregivers

Collaborate with the interdisciplinary team

Maintain accurate and timely documentation

Participate in on‑call rotation as required by the local branch

Qualifications

Graduate of an accredited nursing school with current licensure to practice in the state of operation

One year nursing experience required; 3+ years preferred

Experience in hospice or a similar setting preferred

Knowledge of the hospice philosophy of care

Commitment to clinical and documentation excellence

Must be a licensed driver with an automobile that is insured and in good working order
