Palliative Care Coordinator
Qualification and Education Requirements
High School Diploma or equivalent
Current Driver's License & Reliable Transportation
Must provide ongoing valid and current auto liability insurance
Excellent communication and relationship building skills
Proven experience in healthcare field or community outreach role
Compassionate and empathetic approach to patient care
Ability to work independently and collaboratively within a team
Experience with EMR software is a plus
Essential Duties
Initiate contact with potential clients via phone, email, and in person meetings
Educate potential clients about palliative services available to them through InHome Connect
Follow up and maintain an up-to-date record of interactions
Navigate EMR efficiently to accurately record and communicate the needs and activities of clients and/or patients
Effectively communicates patient needs to the palliative team
Schedules patients for visits / phone calls with appropriate team members
Acquires consent needed to enroll new patients in palliative care
Complies with Medicare, Medicaid, and HIPAA regulations and requirements always.
Performs other duties as required.
