Overview UW Medical Center - Northwest has an outstanding opportunity for a Nurse Care Coordinator to join our Care Management team with the goal of achieving coordinated patient flow through an inpatient stay, culminating in a safe and timely discharge. This position works closely with all members of the interdisciplinary health care team, ensuring that the team is aware of the patient’s plan of care and that all disciplines are working toward the same goals for the patient.

Responsibilities Identification and monitoring of high medical needs patients to ensure timely completion of consults, procedures and tests, ongoing team communication, and facilitates coordination between providers across the continuum of care.
Lead medical team and nursing huddles and work creatively on removing barriers to discharge due to medical complications and/or system or team obstacles.
Assist with ordering complex medical equipment and in ensuring safe transitions of care.
Develop and monitor outcome measures related to care coordination and staff and patient satisfaction with discharge planning and providing ongoing consultation and education to staff on how to achieve improved patient flow through the UWMC system.
Required Position Qualifications Bachelor\'s degree in Nursing or related field. Master\'s preferred.
Three years of professional in-patient or out-patient oncology nursing experience. Experience in Care Management strongly preferred.
Able to communicate effectively in English, both verbally and in writing.
Effective interpersonal skills, leadership skills, and clinical expertise.
Ability to access information and document in electronic medical record.
Current Washington State RN licensure at time of hire, and current BLS.

Compensation and Benefits Pay Range Minimum: $45.59 hourly
Pay Range Maximum: $83.64 hourly
Other Compensation: $10,000 Sign-On Bonus and $5,000 Relocation Assistance; Premium for BSN (or higher degree) $1.00/hour; Specialty Certs Premium $1.25/hour; Weekend Shift Premium $4.00/hour; Overtime Pay
