Job Description: Nurse Practitioner - Palliative Care Location: Jackson, Pike, Scioto, and Gallia counties

Position Overview: We are seeking a dedicated Palliative Care Nurse to join our team and share our vision of enhancing quality of life and care for seniors.
Essential Functions: Conduct physical exams and comprehensive medical and social histories for individuals, including healthy individuals and those with acute or chronic illnesses. Make independent management and treatment decisions.
Order, perform, and interpret laboratory tests for screening and diagnosis.
Prescribe and manage medications, diets, and other treatments such as physical therapy.
Perform therapeutic or corrective procedures, including urgent care.
Refer patients to specialists, nurses, physicians, or other healthcare providers as needed.
Provide education, counseling on health maintenance, promotion, illness prevention, management, and medication use.
Evaluate the effectiveness of interventions and provide additional support as necessary. Follow up with noncompliant patients.
Serve as a preceptor for medical, nursing, NP, or PA students.
Consult with healthcare providers about patients admitted to hospitals, rehab, or nursing facilities.
Coordinate with insurers, employers, government agencies, and other providers regarding patient care.
Develop and monitor care plans, determine need for referrals, hospital admissions, or emergency care, and assess when illnesses are resolved.
Assess social and economic factors influencing patient care and connect patients with community resources.
Establish contracts with clients detailing responsibilities.
Maintain productivity levels to meet organizational goals.
