Schedule: Monday through Friday 8:30 am to 5:00 pm

What You’ll Do / Responsibilities Observe and monitor patient conditions and perform OASIS assessments to develop an individualized care plan and adjust as needs change (OASIS instruction will be provided if not already known).
Plan of Care: develop and adjust care plans, coordinate care, and supervise the patient’s condition.
Minimize Patient Risk: administer medications as prescribed and prioritize visits for high-risk patients to decrease re-hospitalizations.
Supervision: oversee total care provided by nurse aides and LPNs.
Continuity of Care: manage multidisciplinary care, admissions, transfers, and discharges to promote continuity.
Family Educator/Advocate: educate patient and family on the disease process using teach-back methods.
Policies: complete clinical documentation per agency protocol and Medicare/Federal guidelines; collaborate with the team and physicians to ensure optimal care.
Rules and Regulations: follow agency policies and communicate changes in schedule or availability.
Operations: attend in-service training and mandatory agency meetings.

Requirements Registered Nurse with current license in the state of employment.
Minimum one-year RN experience in an acute care setting; home care experience preferred.

Capable of all physical requirements of the role.
