Overview Nurse Practitioner - Palliative Care Hospice – PRN
Absolute Hospice is presently seeking a Nurse Practitioner to join our Hospice Team. Come join our family-owned company!
Education Graduate of an accredited school of Advanced Practice Registered Nursing
End of Life Nursing Education Program (ELNEC) to include all 8 modules must be completed within 18 months of employment
Current Advanced Practice Registered Nurse License within the state of Ohio
MSN or Doctoral Degree required
Certification in Hospice and Palliative Nursing desired
Experience Minimum of one year experience as an Advanced Practice Registered Nurse, end of life care experience preferred
Core Competencies Demonstrated competency in the performance of professional nursing functions
Excellent oral and written communication skills
Ability to work with dying patients and their families
Sympathetic attitude toward caring for the hospice patient; positive communication with the hospice team
Accurately assess the eligibility of patients for hospice service
Other Valid driver\'s license and auto insurance
Summary Of Major Functions Performs comprehensive nursing assessments
Assess for signs and symptoms of discomfort including the presence of pain and take prompt and appropriate action
Initiates plan of care as needed
Coordinates all services provided to patients/families in all settings to ensure quality and appropriateness of care
Provides direct face-to-face visits to referred patients and patients already receiving hospice services; contacts physician for updates on patient\'s eligibility and clinical status
Demonstrates flexibility in scheduling; accepts new admissions and emergency assignments as requested
Maintains knowledge of state and federal regulations and ACHC standards (if applicable)
Participates in QAPI program
Observes safety hazards and takes steps to reduce or eliminate them
Maintains continuing education through academic studies, seminars, and workshops
Attends and participates in in-services, staff meetings, and patient care conferences
Completes all documentation accurately and legibly
Complies with hospice policies, procedures, laws, regulations, and standards of practice
Performs all related duties as assigned
