We are seeking a Registered Nurse to serve patients throughout the Midlands East area. The Nurse Case Manager is a critical part of the patient’s care team. The nurse will visit patients and facilitate care with the appropriate provider. The Nurse Case Manager will travel to patients’ homes, long‑term care facilities, and other sites to evaluate patients and initiate tele‑medicine visits. Provide quality healthcare in adherence to all applicable laws, regulations, and policies within the scope of practice.

The role is full‑time, salary‑based, 8‑hour shifts Monday‑Friday (8am‑5pm). Reliable transportation is required daily. Service area: Midlands East.

Responsibilities

Facilitate receipt of all medical records from the patient’s primary provider and specialists.

Review medical records and complete consents with patients.

Enroll patients in Care Management if they meet eligibility criteria.

Initiate a Care Management Plan of Care if the patient is eligible.

Capture diagnoses at the highest specificity and ensure they are accepted.

Complete AWVs, cognitive impairment screenings, Social Determinants of Health assessments, and ACPs.

Evaluate for home health, hospice, palliative, or consults with specialty divisions.

Evaluate for RPM devices, resources, or tools that may improve the patient’s quality of life.

Communicate and coordinate care, reconcile medications, and provide post‑discharge education.

Evaluate for adaptive equipment, DME, safe environment, and acute condition follow‑up.

Accurately document all Care Management activities and keep care plans up to date.

Qualifications

Licensed registered nurse in good standing with appropriate board/issuer.

Minimum three (3) years of clinical experience preferred.

Experience in community settings preferred.

Strong written and verbal communication skills.

Basic computer knowledge and ability to manage multiple responsibilities.

Licensed driver with an insured automobile in good working order.

Flexibility to work flexible hours and travel between facilities.
