About the Role
The Nurse Case Manager – Home Infusion (Nurse Case Manager – Home IV) coordinates patient care needs between the home infusion pharmacy, home health agency, hospital, physician office, and managed care providers for patients in the home setting. Provides nursing oversight of the patient’s care from admission until the end of therapy and communicates patient needs or changes to the appropriate personnel. Participates in teaching patients and caregivers about proper medical management of prescribed home infusion therapies.

Schedule: Monday – Friday, 8:00 AM – 5:00 PM.

Service Areas: Charleston, SC and surrounding areas in SC. May travel to Columbia, SC and Greenville, SC as needed.

Minimum Qualifications

High School Diploma or Equivalent.

Graduate from an accredited school of nursing.

Active Registered Nurse license in the state(s) of practice.

1 year current clinical nursing experience.

BLS or obtain upon hire.

Proficient with partner programs (i.e.: MS Outlook, Word, Excel).

Ability to cope with ambiguous, changing environment while under pressure in a calm manner.

Must be able to work as a team member and develop productive and cooperative working relationships with all members of the Palmetto Infusion team.

Essential Functions

Provides infusion and other skilled nursing care to patients including, but not limited to: patient assessment and reassessment; patient and caregiver education/teaching; development and revision of the Plan of Care and the Plan of Treatment; implementation of physician orders; monitoring patient response; response to emergency patient needs; psychosocial support.

Provides support and coordinated marketing efforts with sales team.

Completes all required patient care documentation and reports accurately.

Communicates changes in patient condition to supervisor, physicians and other health care providers as appropriate.

Maintains knowledge and proficiency in medical practices through continuing education, staff meetings, workshops, and home study.

Participates in the orientation of other staff as requested.

Takes on-call responsibility as assigned to provide 24-hour availability according to protocols.

Participates in Performance Improvement processes.

Complies with all applicable laws, regulations, and PI policies and personnel qualifications.
