Position Summary
Responsible for enrolling and assessing clients and providing services to those clients enrolled in the program with a focus on Social Determinants of Health (SDOH). Maintains detailed and accurate records, including documenting all contact with each client, and keeping the client’s primary care provider informed of the client’s progress and any barriers the client may be facing regarding SDOH services.
Duties and Responsibilities
Assesses client’s needs and provides services accordingly.
Assesses SDOH services for each client.
Coordinates treatment adherence services with medical providers and other relevant staff.
Recruits and enrolls clients who need living assistance, legal services, and food assistance.
Refers clients to health education services.
Participates in the organization, implementation and monitoring of grant deliverables, including attending meetings as appropriate, and collecting, reviewing, and producing educational materials.
Refers clients to other service providers at CHN and externally.
Improves access to primary care for treatment adherence clients.
Provides appropriate documentation of encounters, case conferences, assessment, and reassessment.
Enters data appropriately into charts, medical records, and other systems on a consistent and timely basis.
Facilitates related services for in‑reach and outreach for health center patients and CHN staff as appropriate.
Maintains competency by attending workshops, conferences, and seminars.
Compliance with Employee Health Services.
Performs other duties, as assigned.
Education & Experience Required
High School Diploma or GED Equivalent required.
Associate degree preferred.
Experience working with HIV and current with the latest HIV treatment advancements.
Knowledge, Skills and Abilities Required
Computer proficient, with experience in WordPerfect, Word, Excel, and data entry.
Strong verbal and written communication skills.
Strong interpersonal skills.
Ability to organize and prioritize.
Interest in and conviction of purpose and aims for all programs in the clinic.
Respect for the rights of individuals to make their own decisions about their medical care.
Ability to communicate easily and display a cordial manner towards individuals from a variety of socio-economic, cultural, and religious backgrounds.
Bilingual Spanish.
