Our MJHS Medical Associates, P.C. is a team of Nurse Practitioners, Physician Assistants, RN Case Managers, and LPNs who provide care to Elderplan members residing in assisted living and long-term care facilities, as well as those living at home.
Ensure high-quality, cost-effective care and services for Elderplan members through support of professional Care Management and/or Clinical Service activities.
This position supports all aspects of care coordination for our ISNP, IESNP, and Elderplan Plus members in compliance with departmental and regulatory requirements.
Receive and process assignments from ISNP Case Managers.
Receive and process assignments from Community staff/PCP outreach.
High School Diploma or equivalent; College Degree preferred.
One-year prior managed care experience preferred.
Prior experience in a healthcare setting preferred.
Familiarity with utilization management and case management preferred.
