Overview Lead Medical Case Manager role at Positive Impact Health Centers (PIHC). PIHC is a community leader in providing HIV prevention, care and treatment services, offering access to medical, pharmacy, dental, behavioral health and social services to help patients achieve high-quality health outcomes.
Job Summary The Lead Medical Case Manager (LMCM) coordinates medical case management duties for the respective care team and provides direct supervision for Medical Case Managers within their assigned PIHC Center. Duties include ensuring quality support services to clients, overseeing staff training, and ensuring required standards and guidelines are in place. The LMCM also provides direct case management services to assigned patients within the care team.
Essential Functions Learn and remain current on practice issues related to HIV/AIDS, related medical diagnoses, and medical case management.
Provide medical case management services to identified patients following HRSA/Ryan White and Metropolitan Atlanta HIV Health Services Planning Council guidelines.
Instruct and supervise assigned staff regarding federal, state, health district, and agency standards and policies.
Ensure assigned staff perform day-to-day operations, including coverage of the departmental Resource Line and scheduling patient enrollment visits.
Provide medical case management for an average caseload of 40 patients; develop an Individualized Service Plan (ISP).
Assign clinic patients to Medical Case Managers for medical and non-medical services, using Acuity Level scores and service needs while maintaining equitable caseloads.
Participate in weekly interdisciplinary Case Conference meetings, leading when needed; participate in daily team huddles and assist with documentation of summaries.
Collaborate with Site Coordinator and Data Team to collect data and review reports to measure patient acuity, health care outcomes, and staff productivity.
Assist with orientation of new medical case managers at the designated PIHC Center.
Consult with interdisciplinary care team regarding patients’ ongoing need for care and referrals.
Assist Site Coordinator with managing the EMR Medical Case Management pool and assign clients accordingly.
Document patient information, activities, referrals and consultations in the EMR system and related reporting software in a timely and accurate manner.
Abide by all applicable laws and agency policies related to confidentiality and HIPAA.
Organize and lead weekly team meetings; monitor time and attendance in the payroll system; complete performance evaluations; comply with conflict of interest policies.

Minimum Qualifications Master’s degree in social work, human services, or public health.
Basic computer skills.
Experience Minimum one-year experience in social work or related service delivery field.
One year of professional experience in an HIV service setting preferred.
Administrative or supervisory experience preferred.
Experience documenting in an Electronic Medical Record (EMR) system required.
License/Licensure (if Eligible) Licensure, if applicable to degree held (e.g., LMSW or LCSW for MSW), is encouraged.
