This program provides supports to adults living in permanent subsidized housing to promote housing stability, wellness, independence, recovery, and community integration. Housing stability is measured by ongoing housing, decreased hospitalizations/ER visits, fewer crisis episodes, and participation in maintenance of personal housing stability and wellness. Supports are tailored to the individual's needs to promote resiliency and address the effects of SPMI and lived trauma. The Housing Support staff serves as the first point of contact for landlords/property owners for issues arising with individuals in supportive housing and provides linkage to community resources and services.
Responsibilities Carry a caseload (25-35 clients) and conduct a minimum of 1 visit per week per client within the residence or community setting for the first three months after housing is obtained. After three months, individuals are required to be seen twice per month.
Provide care coordination to ensure housing stability and independent living skills; facilitate referrals and linkages to internal/external community services and providers to assure a continuum of services.
Provide ongoing support, case management, and skills training aligned with the treatment plan to help clients achieve goals.
Gather information from clients, family members, and support systems to ensure appropriate level of care.
Recognize the need for crisis intervention and notify supervisor/clinical contact in escalated situations.
Complete timely, high-quality documentation for all service contacts (within 24 hours) to meet agency and state standards.
Participate in weekly team meetings to provide input for client treatment needs.
Meet agency productivity standard of 40 billable hours per month.
Meet regularly with program manager for professional growth and performance evaluation.
Perform additional duties as assigned by team lead/program manager/director.
Minimum Qualifications Bachelor’s degree in a related field (e.g., human services, psychology, counseling, criminal justice, sociology, social work) or equivalent education and experience.
One year of experience in a behavioral health setting providing case management or similar.
Proficient technology skills to use electronic medical records, computer, telehealth system, and mobile devices.
Care coordination skills to link individuals to resources within the agency and community.
Must have transportation to and from work, strong interpersonal skills, and be a positive team player.
Working Conditions Community setting, Monday–Friday 8:15am–5:00pm; some evenings and weekend hours may be required, including crisis line rotation.
In-person service delivery.
Travel throughout DBHDD Region 3 required.
Ability to use a computer, telephone, fax, copier, and scanning equipment.
Must pass pre-employment drug screen and background check; valid Georgia driver’s license and motor vehicle record; complete agency trainings as required.
